After reviewing the case, please write a single 1 hour session plan with at least 4 interventions. In your session plan write-up, please include all needed materials, the steps for your intervention, target behaviors, and response definitions. Please provide a 3-5 sentence rationale for the written intervention.
The patient is a 45-year-old woman named Claire who has been living with chronic lower back pain for the past five years. The pain began following a car accident but has persisted despite physical therapy, medications, and lifestyle modifications. Claire describes the pain as a constant, dull ache, with occasional flare-ups that severely limit her mobility and daily activities. She works as a graphic designer, but the pain has made it difficult for her to maintain long hours in front of the computer, leading to increased work-related stress.
In addition to her physical pain, Claire has been experiencing symptoms of depression for the last year. She feels increasingly isolated, has lost interest in activities she once enjoyed (such as painting and going for walks), and often feels hopeless about her condition. She describes feeling trapped in a cycle of pain and depression, where her mood worsens as her physical symptoms worsen, and vice versa.
Claire’s physician referred her to a multidisciplinary pain management team, where a music therapist was included to help address both her physical discomfort and emotional distress. During the initial consultation, Claire mentioned that she enjoys indie folk and acoustic music and used to play the guitar as a hobby, but she no longer has the energy or motivation to do so. She also shared that she finds it hard to relax and often feels overwhelmed by her inability to enjoy life as she once did.

After reviewing the case, please write a 6-week treatment plan for the client. Each week should be labeled with the specific goal for the week, along with corresponding objectives. From these 6 weeks, choose at least 3 weeks to include one intervention for each session. For each intervention, the write-up should include: Materials needed, steps for the intervention, target behaviors and response definitions. At the bottom of your plan, provide a 5-8 sentence rationale explaining how your interventions address the selected goals and objectives, and how the chosen goals align with the client's needs.
Ethan is a 14-year-old boy diagnosed with Autism Spectrum Disorder (ASD), Level 3, which is characterized by severe impairments in communication, social interaction, and behavior. Ethan has significant difficulty with verbal communication, often using only a few words or gestures to express his needs. He is nonspeaking in many situations and relies heavily on a few words or signs to convey his thoughts. Ethan experiences frequent emotional dysregulation, including intense meltdowns and aggressive behaviors, which are triggered by changes in routine, sensory overload, or frustration with communication difficulties.
In the past year, Ethan has become increasingly withdrawn and anxious. He avoids social interactions, refuses to participate in group activities, and exhibits high levels of social anxiety. Ethan's parents have noticed that he has been isolating himself in his room, often engaging in repetitive behaviors such as hand-flapping or pacing. He has also shown signs of depression, including a lack of interest in activities he once enjoyed, such as playing with toys or going outside. He frequently expresses feelings of hopelessness and frustration, as he struggles to communicate his emotions effectively and cannot understand why he feels so overwhelmed.
Due to the worsening of his emotional and behavioral challenges, and the inability to manage these symptoms at home, Ethan was admitted to a residential mental health facility. The facility provides intensive care, including a multidisciplinary approach to address Ethan's communication difficulties, emotional regulation, and behavioral concerns.
